Church Forms

One of the joys of belonging to a church family is Church Membership. We have a 4 page packet of information for WCAG church attenders who desire to become members. If you are interested in church membership, print the forms, fill them out and give them to one of the pastors the next time you are at the church. WCAG offers two scholarships for students. Download the Application Form here.

Calendar/Facilities forms are used to put a calendar date on the official WCAG calendar and to reserve rooms and equipment. Please read the 3rd page titled "West County Assembly of God - Facilities Guidelines". If you have any questions about filling out the forms, please call Joan Newton, 314-434-1058 or email her joan-n@wcag.org. Purchase Order forms are used by Department Heads, Staff members and Employees to get expenditures pre-approved. Expenditures for any department over $100. must be pre-approved.
